Nomination Form

The gold medallion for the State of Oklahoma, was created by former executive director Betty Price and introduced at the 1989 ceremonies. The medallion features a detail of the Capitol sculpture entitled, As Long as the Waters Flow by Allan Houser, a Chiricahua Apache who was born in Oklahoma and achieved world-acclaim as an artist before his death in 1994. It is a symbol of the centuries-old presence and contribution of Native Americans to Oklahoma.

The Oklahoma Arts Council is the official state agency for the support and development of the arts in Oklahoma.
